Kotetsu tenses at being approached but actually freezes a moment when spoken to. Of course, Emet-Selch has indeed found perhaps the most pathetic of all pathetically kindhearted people in the form of this new Pleroma. By the time Emet-Selch is done soaking with the guard's voice, Kotetsu furrows his brow and the tension takes on a more proactive type of aura.
He's here to help the other Pleroma if they need it but someone asking directly for his help is not a thing he can ignore since the other Pleroma have their plans, anyway. Since he's not part of that plan, he feels no hesitance to deviate momentary from his intentions to help others. It really doesn't matter if it's an enemy guard or not.
Of course, Kotetsu has no clue why out of all the people here he's the one approached, but that is brushed off to the side as unimportant. Even if he did consider it seriously, his deductive conclusion from that would be so far off the mark that it's not even funny. "You okay? What's wrong?"
At the very least Kotetsu still seems wary of the guard, but certainly not nearly enough as would be smart in enemy territory.
